Subject: Application for leave of two weeks
Respected Sir/Madam,
I wish to inform you that I have been advised by my doctor to undergo an urgent surgery for a medical problem. The surgery is scheduled for 14 August 2017, and I will need complete rest for about two weeks after it.
I therefore request you to kindly grant me leave for two weeks, from 14 August 2017 to 27 August 2017. I have handed over my current work to my colleague, Mr. Arjun Rao, who will look after it in my absence. I shall be available on phone for any urgent matter. A copy of the doctor’s certificate is enclosed.
